The Injury Reporting and Care Process

1. The Injury Reporting
Every job-related injury should be reported as soon as possible to your supervisor. Legally, ‘Injury’ also includes any illness or disease that is caused or aggravated by the employment, as well as damage to medical braces, artificial limbs, and other prosthetic devices.
2. Obtain Medical Care
Before obtaining medical treatment, ask your supervisor to authorize care using Form CA-16. You have the right to initially select the physician to provide the necessary treatment. Be sure to bring Form CA-16 and Form OWCP-1500/HCFA-1500 to the provider you select. Please note: for occupational disease claims, CA-16 may not be issued without prior approval from OWCP. Emergency medical treatment may be obtained without prior authorization.
